After commit f7fac17ca925 ("xhci: Convert xhci_handshake() to use
readl_poll_timeout_atomic()"), ASMedia xHCI may fail to suspend.
Although the algorithms are essentially the same, the old max timeout is
(usec + usec * time of doing readl()), and the new max timeout is just
usec, which is much less than the old one.
Increase the timeout to make ASMedia xHCI able to suspend again.

The system can hit a deadlock if an xhci adapter breaks while initializing.
The deadlock is between two threads: thread 1 is tearing down the
adapter and is stuck in usb_unlocked_disable_lpm waiting to lock the
hcd->handwidth_mutex. Thread 2 is holding this mutex (while still trying
to add a usb device), but is stuck in xhci_endpoint_reset waiting for a
stop or config command to complete. A reboot is required to resolve.
It turns out when calling xhci_queue_stop_endpoint and
xhci_queue_configure_endpoint in xhci_endpoint_reset, the return code is
not checked for errors. If the timing is right and the adapter dies just
before either of these commands get issued, we hang indefinitely waiting
for a completion on a command that didn't get issued.
This wasn't a problem before the following fix because we didn't send
commands in xhci_endpoint_reset:

With the patch I am submitting, a duration test which breaks adapters
during initialization (and which deadlocks with the standard kernel) runs
without issue.

Early xHCI 1.1 spec did not mention USB 3.1 capable hosts should set
sbrn to 0x31, or that the minor revision is a two digit BCD
containing minor and sub-minor numbers.
This was later clarified in xHCI 1.2.
Some USB 3.1 capable hosts therefore have sbrn set to 0x30, or minor
revision set to 0x1 instead of 0x10.
Detect the USB 3.1 capability correctly for these hosts as well

If an endpoint is encountered that returns USB3_LPM_DEVICE_INITIATED, keep
checking further endpoints, as there might be periodic endpoints later
that return USB3_LPM_DISABLED due to shorter service intervals.
Without this, the code can set too high a maximum-exit-latency and
prevent the use of multiple USB3 cameras that should be able to work.

If host/hub initiated link pm is prevented by a driver flag we still must
ensure that periodic endpoints have longer service intervals than link pm
exit latency before allowing device initiated link pm.
Fix this by continue walking and checking endpoint service interval if
xhci_get_timeout_no_hub_lpm() returns anything else than USB3_LPM_DISABLED
While at it fix the split line error message

The check printing out the "WARN Wrong bounce buffer write length:"
uses incorrect values when comparing bytes written from scatterlist
to bounce buffer. Actual copied lengths are fine.
The used seg->bounce_len will be set to equal new_buf_len a few lines later
in the code, but is incorrect when doing the comparison.
The patch which added this false warning was backported to 4.8+ kernels
so this should be backported as far as well.

The Falcon microcontroller that runs the XUSB firmware and which is
responsible for exposing the XHCI interface can address only 40 bits of
memory. Typically that's not a problem because Tegra devices don't have
enough system memory to exceed those 40 bits.
However, if the ARM SMMU is enable on Tegra186 and later, the addresses
passed to the XUSB controller can be anywhere in the 48-bit IOV address
space of the ARM SMMU. Since the DMA/IOMMU API starts allocating from
the top of the IOVA space, the Falcon microcontroller is not able to
load the firmware successfully.
Fix this by setting the DMA mask to 40 bits, which will force the DMA
API to map the buffer for the firmware to an IOVA that is addressable by
the Falcon.

ohci_shutdown() disables all the interrupt and rh_state is set to
OHCI_RH_HALTED. In other hand, ohci_irq() is possible to enable
OHCI_INTR_SF and OHCI_INTR_MIE on ohci_irq(). Note that OHCI_INTR_SF
is possible to be set by start_ed_unlink() which is called:
ohci_irq()
-> process_done_list()
-> takeback_td()
-> start_ed_unlink()
So, ohci_irq() has the following condition, the issue happens by
&ohci->regs->intrenable = OHCI_INTR_MIE | OHCI_INTR_SF and
ohci->rh_state = OHCI_RH_HALTED:

To fix the issue, ohci_shutdown() holds the spin lock while disabling
the interruption and changing the rh_state flag to prevent reenable
the OHCI_INTR_MIE unexpectedly. Note that io_watchdog_func() also
calls the ohci_shutdown() and it already held the spin lock, so that
the patch makes a new function as _ohci_shutdown().
This patch is inspired by a Renesas R-Car Gen3 BSP patch
from Tho Vu.

When a USB device is connected to the host controller and
the system enters suspend, the following error happens
in xhci_suspend():
xhci-hcd ee000000.usb: WARN: xHC CMD_RUN timeout
Since the firmware/internal CPU control the USBSTS.STS_HALT
and the process speed is down when the roothub port enters U3,
long delay for the handshake of STS_HALT is neeed in xhci_suspend().
So, this patch adds to set the XHCI_SLOW_SUSPEND.

The AMD PLL USB quirk is incorrectly enabled on newer Ryzen
chipsets. The logic in usb_amd_find_chipset_info currently checks
for unaffected chipsets rather than affected ones. This broke
once a new chipset was added in e788787ef. It makes more sense
to reverse the logic so it won't need to be updated as new
chipsets are added. Note that the core of the workaround in
usb_amd_quirk_pll does correctly check the chipset.

A USB3 device needs to be reset and re-enumarated if the port it
connects to goes to a error state, with link state inactive.
There is no use in trying to recover failed transactions by resetting
endpoints at this stage. Tests show that in rare cases, after multiple
endpoint resets of a roothub port the whole host controller might stop
completely.
Several retries to recover from transaction error can happen as
it can take a long time before the hub thread discovers the USB3
port error and inactive link.
We can't reliably detect the port error from slot or endpoint context
due to a limitation in xhci, see xhci specs section 4.8.3:
"There are several cases where the EP State field in the Output
Endpoint Context may not reflect the current state of an endpoint"
and
"Software should maintain an accurate value for EP State, by tracking it
with an internal variable that is driven by Events and Doorbell accesses"
Same appears to be true for slot state.
set a flag to the corresponding slot if a USB3 roothub port link goes
inactive to prevent both queueing new URBs and resetting endpoints.

USB 3.2 capability in a host can be detected from the
xHCI Supported Protocol Capability major and minor revision fields.
If major is 0x3 and minor 0x20 then the host is USB 3.2 capable.
For USB 3.2 capable hosts set the root hub lane count to 2.
The Major Revision and Minor Revision fields contain a BCD version number.
The value of the Major Revision field is JJh and the value of the Minor
Revision field is MNh for version JJ.M.N, where JJ = major revision number,
M - minor version number, N = sub-minor version number,
e.g. version 3.1 is represented with a value of 0310h.
Also fix the extra whitespace printed out when announcing regular
SuperSpeed hosts.

In xhci_debugfs_create_slot(), kzalloc() can fail and
dev->debugfs_private will be NULL.
In xhci_debugfs_create_endpoint(), dev->debugfs_private is used without
any null-pointer check, and can cause a null pointer dereference.
To fix this bug, a null-pointer check is added in
xhci_debugfs_create_endpoint().
This bug is found by a runtime fuzzing tool named FIZZER written by us.

The failing case is when disabling LPM. So it is sufficient to avoid

Xhci_handshake() implements the algorithm already captured by
readl_poll_timeout_atomic(). Convert the former to use the latter to
avoid repetition.
Turned out this patch also fixes a bug on the AMD Stoneyridge platform
where usleep(1) sometimes takes over 10ms.
This means a 5 second timeout can easily take over 15 seconds which will
trigger the watchdog and reboot the system.

This change fixes a data corruption issue occurred on USB hard disk for
the case that bounce buffer is used during transferring data.
While updating data between sg list and bounce buffer, current
implementation passes mapped sg number (urb->num_mapped_sgs) to
sg_pcopy_from_buffer() and sg_pcopy_to_buffer(). This causes data
not get copied if target buffer is located in the elements after
mapped sg elements. This change passes sg number for full list to
fix issue.
Besides, for copying data from bounce buffer, calling dma_unmap_single()
on the bounce buffer before copying data to sg list can avoid cache issue.

Commit 2f31a67f01a8 ("usb: xhci: Prevent bus suspend if a port connect
change or polling state is detected") was intended to prevent ports that
were still link training from being forced to U3 suspend state mid
enumeration.
This solved enumeration issues for devices with slow link training.
Turns out some devices are stuck in the link training/polling state,
and thus that patch will prevent suspend completely for these devices.
This is seen with USB3 card readers in some MacBooks.
Instead of preventing suspend, give some time to complete the link
training. On successful training the port will end up as connected
and enabled.
If port instead is stuck in link training the bus suspend will continue
suspending after 360ms (10 * 36ms) timeout (tPollingLFPSTimeout).
Original patch was sent to stable, this one should go there as well

A suspended SS port in U3 link state will go to U0 when resumed, but
can almost immediately after that enter U1 or U2 link power save
states before host controller driver reads the port status.
Host controller driver only checks for U0 state, and might miss
the finished resume, leaving flags unclear and skip notifying usb
code of the wake.
Add U1 and U2 to the possible link states when checking for finished
port resume.

The function r8a66597_endpoint_disable() and r8a66597_urb_enqueue() may
be concurrently executed.
The two functions both access a possible shared variable "hep->hcpriv".
This shared variable is freed by r8a66597_endpoint_disable() via the
call path:

The read operation is protected by a spinlock, but the free operation
is not protected by this spinlock, thus a concurrency use-after-free bug
may occur.
To fix this bug, the spin-lock and spin-unlock function calls in
r8a66597_endpoint_disable() are moved to protect the free operation.

As commented in the struct's definition there shouldn't be anything
underneath its 'priv[0]' member as it would break some macros.
The patch converts the broken_suspend into a bit-field and relocates it
next to to the rest of bit-fields.

The code to prevent a bus suspend if a USB3 port was still in link training
also reacted to USB2 port polling state.
This caused bus suspend to busyloop in some cases.
USB2 polling state is different from USB3, and should not prevent bus
suspend.
Limit the USB3 link training state check to USB3 root hub ports only.
The origial commit went to stable so this need to be applied there as well

Don't allow USB3 U1 or U2 if the latency to wake up from the U-state
reaches the service interval for a periodic endpoint.
This is according to xhci 1.1 specification section 4.23.5.2 extra note:
"Software shall ensure that a device is prevented from entering a U-state
where its worst case exit latency approaches the ESIT."
Allowing too long exit latencies for periodic endpoint confuses xHC
internal scheduling, and new devices may fail to enumerate with a
"Not enough bandwidth for new device state" error from the host.

Occasionally AMD SNPS 3.0 xHC does not respond to
CSS when set, also it does not flag anything on SRE and HCE
to point the internal xHC errors on USBSTS register. This stalls
the entire system wide suspend and there is no point in stalling
just because of xHC CSS is not responding.
To work around this problem, if the xHC does not flag
anything on SRE and HCE, we can skip the CSS
timeout and allow the system to continue the suspend. Once the
system resume happens we can internally reset the controller
using XHCI_RESET_ON_RESUME quirk
