Users can initiate resets to specific SCSI device/target/host through
IOCTL. When this happens, the SCSI cmd passed to eh_device/target/host
_reset_handler() callbacks is initialized with a request whose tag is -1.
In this case it is not right for eh_device_reset_handler() callback to
count on the LUN get from hba->lrb[-1]. Fix it by getting LUN from the SCSI
device associated with the SCSI cmd.

If someone plays with the UFS clk scaling devfreq governor through sysfs,
ufshcd_devfreq_scale may be called even when HBA is not runtime ACTIVE.
This can lead to unexpected error. We cannot just protect it by calling
pm_runtime_get_sync() because that may cause a race condition since HBA
runtime suspend ops need to suspend clk scaling. To fix this call
pm_runtime_get_noresume() and check HBA's runtime status. Only proceed if
HBA is runtime ACTIVE, otherwise just bail.

If UFS host device is in runtime-suspended state while UFS shutdown
callback is invoked, UFS device shall be resumed for register
accesses. Currently only UFS local runtime resume function will be invoked
to wake up the host. This is not enough because if someone triggers
runtime resume from block layer, then race may happen between shutdown and
runtime resume flow, and finally lead to unlocked register access.
To fix this, in ufshcd_shutdown(), use pm_runtime_get_sync() instead of
resuming UFS device by ufshcd_runtime_resume() "internally" to let runtime
PM framework manage the whole resume flow.

The scsi_block_reqs_cnt increased in ufshcd_hold() is supposed to be
decreased back in ufshcd_ungate_work() in a paired way. However, if
specific ufshcd_hold/release sequences are met, it is possible that
scsi_block_reqs_cnt is increased twice but only one ungate work is
queued. To make sure scsi_block_reqs_cnt is handled by ufshcd_hold() and
ufshcd_ungate_work() in a paired way, increase it only if queue_work()
returns true.

If somehow no interrupt notification is raised for a completed request and
its doorbell bit is cleared by host, UFS driver needs to cleanup its
outstanding bit in ufshcd_abort(). Otherwise, system may behave abnormally
in the following scenario:
After ufshcd_abort() returns, this request will be requeued by SCSI layer
with its outstanding bit set. Any future completed request will trigger
ufshcd_transfer_req_compl() to handle all "completed outstanding bits". At
this time the "abnormal outstanding bit" will be detected and the "requeued
request" will be chosen to execute request post-processing flow. This is
wrong because this request is still "alive".

In ufshcd_suspend(), after clk-gating is suspended and link is set
as Hibern8 state, ufshcd_hold() is still possibly invoked before
ufshcd_suspend() returns. For example, MediaTek's suspend vops may
issue UIC commands which would call ufshcd_hold() during the command
issuing flow.
Now if UFSHCD_CAP_HIBERN8_WITH_CLK_GATING capability is enabled,
then ufshcd_hold() may enter infinite loops because there is no
clk-ungating work scheduled or pending. In this case, ufshcd_hold()
shall just bypass, and keep the link as Hibern8 state.

Urgent bkops level is used to compare against actual bkops status read from
UFS device. Urgent bkops level is set during initialization and might be
updated in exception event handler during runtime. But it should not be
updated to the actual bkops status every time when auto bkops is toggled.
Otherwise, if urgent bkops level is updated to 0, auto bkops shall always
be kept enabled.

The async version of ufshcd_hold(async == true), which is only called in
queuecommand path as for now, is expected to work in atomic context, thus
it should not sleep or schedule out. When it runs into the condition that
clocks are ON but link is still in hibern8 state, it should bail out
without flushing the clock ungate work.

Auto-Hibern8 may be disabled by some vendors or sysfs in runtime even if
Auto-Hibern8 capability is supported by host. If Auto-Hibern8 capability is
supported by host but not actually enabled, Auto-Hibern8 error shall not
happen.
To fix this, provide a way to detect if Auto-Hibern8 is actually enabled
first, and bypass Auto-Hibern8 disabling case in
ufshcd_is_auto_hibern8_error().

Pass UFS device information to vendor-specific variant callback
"apply_dev_quirks" because some platform vendors need to know such
information to apply special handling or quirks in specific devices.
At the same time, modify existing vendor implementations according to the
new interface for those vendor drivers which will be built-in or built as a
module alone with UFS core driver.

In UFS host reset and restore path, before probe, we stop and start the
host controller once. After host controller is stopped, the pending
requests, if any, are cleared from the doorbell, but no completion IRQ
would be raised due to the hba is stopped. These pending requests shall be
completed along with the first NOP_OUT command (as it is the first command
which can raise a transfer completion IRQ) sent during probe. Since the
OCSs of these pending requests are not SUCCESS (because they are not yet
literally finished), their UPIUs shall be dumped. When there are multiple
pending requests, the UPIU dump can be overwhelming and may lead to
stability issues because it is in atomic context. Therefore, before probe,
complete these pending requests right after host controller is stopped and
silence the UPIU dump from them.

ufshcd_scsi_add_wlus() fails

A non-zero error value likely being returned by ufshcd_scsi_add_wlus() in
case of failure of adding the WLs, but ufshcd_probe_hba() doesn't use this
value, and doesn't report this failure to upper caller. This patch is to
fix this issue.

bkops level should be rechecked upon receiving an exception. Currently the
level is being cached and never updated.
Update bkops each time the level is checked. Also do not use the cached
bkops level value if it is disabled and then enabled.

During clock gating (ufshcd_gate_work()), we first put the link hibern8 by
calling ufshcd_uic_hibern8_enter() and if ufshcd_uic_hibern8_enter()
returns success (0) then we gate all the clocks. Now let’s zoom in to what
ufshcd_uic_hibern8_enter() does internally: It calls
__ufshcd_uic_hibern8_enter() and if failure is encountered, link recovery
shall put the link back to the highest HS gear and returns success (0) to
ufshcd_uic_hibern8_enter() which is the issue as link is still in active
state due to recovery! Now ufshcd_uic_hibern8_enter() returns success to
ufshcd_gate_work() and hence it goes ahead with gating the UFS clock while
link is still in active state hence I believe controller would raise UIC
error interrupts. But when we service the interrupt, clocks might have
already been disabled!
This change fixes for this by returning failure from
__ufshcd_uic_hibern8_enter() if recovery succeeds as link is still not in
hibern8, upon receiving the error ufshcd_hibern8_enter() would initiate
retry to put the link state back into hibern8.

In function __ufshcd_query_descriptor(), in the event of an error
happening, we directly goto out_unlock and forget to invaliate
hba->dev_cmd.query.descriptor pointer. This results in this pointer still
valid in ufshcd_copy_query_response() for other query requests which go
through ufshcd_exec_raw_upiu_cmd(). This will cause __memcpy() crash and

In some cases, hba may go through shutdown flow without successful
initialization and then make system hang.
For example, if ufshcd_change_power_mode() gets error and leads to
ufshcd_hba_exit() to release resources of the host, future shutdown flow
may hang the system since the host register will be accessed in unpowered
state.
To solve this issue, simply add checking to skip shutdown for above kind of
situation.

Some UFS memory devices needs their reset line toggled in order to get them
into a good state for initialization. Provide a new vops to allow the
platform driver to implement this operation.

Currently bits in hba->outstanding_tasks are cleared only after their
corresponding task management commands are successfully done by
__ufshcd_issue_tm_cmd().
If timeout happens in a task management command, its corresponding bit in
hba->outstanding_tasks will not be cleared until next task management
command with the same tag used successfully finishes.
This is wrong and can lead to some issues, like power issue. For example,
ufshcd_release() and ufshcd_gate_work() will do nothing if
hba->outstanding_tasks is not zero even if both UFS host and devices are
actually idle.
Solution is referred from error handling of device commands: bits in
hba->outstanding_tasks shall be cleared regardless of their execution
results.

Voltage regulators are optional, so functions must check they exist
before dereferencing.
Note this issue is hidden if CONFIG_REGULATORS is not set, because the
offending code is optimised away.
Notes for stable:
The issue first appears in commit 57d104c153d3 ("ufs: add UFS power
management support") but is inadvertently fixed in commit 60f0187031c0
("scsi: ufs: disable vccq if it's not needed by UFS device") which in
turn was reverted by commit 730679817d83 ("Revert "scsi: ufs: disable vccq
if it's not needed by UFS device""). So fix applies v3.18 to v4.5 and
v5.1+

This patch is to change msleep() to usleep_range() based on
Documentation/timers/timers-howto.txt. It suggests using usleep_range()
for small msec(1ms - 20ms) since msleep() will often sleep longer than
desired value.
After changing, booting time will be 5ms-10ms faster than before. I
tested this change on two different platforms, one has 5ms faster, another
one is about 10ms. I think this is different on different platform.
Actually, from UFS host side, 1ms-5ms delay is already sufficient for its
initialization of the local UIC layer.

Currently only "interrupt-based" errors have their own history, however
there are some "non-interrupt-based" errors or events which need history
to improve debugging or help know the health status of UFS devices.
Example of fatal errors:
- Link startup error
- Suspend error
- Resume error
Example of abnormal events:
- Task or request abort
- Device reset (now equals to Logical Unit Reset)
- Host reset
This patch tries to track above errors and events by existed UFS error
history mechanism.

Currently UFS error history will be reset and lost during host reset flow
by ufschd_probe_hba().
Avoid resetting it so error history can be kept as complete as possible to
improve debugging.
In addition, fix a minor display error in ufshcd_print_err_hist().

"This is mostly update of the usual drivers: qla2xxx, hpsa, lpfc, ufs,
mpt3sas, ibmvscsi, megaraid_sas, bnx2fc and hisi_sas as well as the
removal of the osst driver (I heard from Willem privately that he
would like the driver removed because all his test hardware has
failed). Plus number of minor changes, spelling fixes and other
trivia.
The big merge conflict this time around is the SPDX licence tags.
Following discussion on linux-next, we believe our version to be more
accurate than the one in the tree, so the resolution is to take our
version for all the SPDX conflicts"
Note on the SPDX license tag conversion conflicts: the SCSI tree had
done its own SPDX conversion, which in some cases conflicted with the
treewide ones done by Thomas & co.
In almost all cases, the conflicts were purely syntactic: the SCSI tree
used the old-style SPDX tags ("GPL-2.0" and "GPL-2.0+") while the
treewide conversion had used the new-style ones ("GPL-2.0-only" and
"GPL-2.0-or-later").
In these cases I picked the new-style one.
In a few cases, the SPDX conversion was actually different, though. As
explained by James above, and in more detail in a pre-pull-request
thread:
"The other problem is actually substantive: In the libsas code Luben
Tuikov originally specified gpl 2.0 only by dint of stating:
* This file is licensed under GPLv2.
In all the libsas files, but then muddied the water by quoting GPLv2
verbatim (which includes the or later than language). So for these
files Christoph did the conversion to v2 only SPDX tags and Thomas
converted to v2 or later tags"
So in those cases, where the spdx tag substantially mattered, I took the
SCSI tree conversion of it, but then also took the opportunity to turn
the old-style "GPL-2.0" into a new-style "GPL-2.0-only" tag.
Similarly, when there were whitespace differences or other differences
to the comments around the copyright notices, I took the version from
the SCSI tree as being the more specific conversion.
Finally, in the spdx conversions that had no conflicts (because the
treewide ones hadn't been done for those files), I just took the SCSI
tree version as-is, even if it was old-style. The old-style conversions
are perfectly valid, even if the "-only" and "-or-later" versions are
perhaps more descriptive.

Currently auto-hibernate is activated if host supports auto-hibern8
capability. However error-handling is not implemented, which makes the
feature somewhat risky.
If either "Hibernate Enter" or "Hibernate Exit" fail during auto-hibernate
flow, the corresponding interrupt "UIC_HIBERNATE_ENTER" or
"UIC_HIBERNATE_EXIT" shall be raised according to UFS specification.
This patch adds auto-hibernate error-handling:
- Monitor "Hibernate Enter" and "Hibernate Exit" interrupts after
auto-hibernate feature is activated.
- If a failure happens, trigger error-handling just like
"manual-hibernate" failure and apply the same recovery flow: schedule
UFS error handler in ufshcd_check_errors(), and then do host reset and
restore in UFS error handler.

struct ufs_dev_cmd is the main container that supports device management
commands. In the case of a read descriptor request, we assume that the
proper space was allocated in dev_cmd to hold the returning descriptor.
This is no longer true, as there are flows that doesn't use dev_cmd for
device management requests, and was wrong in the first place.

If UFS device responds an unknown request response code, we can not know
what it was via logs because the code is replaced by "DID_ERROR << 16"
before log printing.
Fix this to provide precise request response code information for easier
issue breakdown.

Currently if a regulator has "<name>-fixed-regulator" property in device
tree, it will skip current limit initialization. This lead to a zero
"max_uA" value in struct ufs_vreg.
However, "regulator_set_load" operation shall be required on regulators
which have valid current limits, otherwise a zero "max_uA" set by
"regulator_set_load" may cause unexpected behavior when this regulator is
enabled or set as high power mode.
Similarly, in device's icc_level configuration flow, the target icc_level
shall be updated if regulator also has valid current limit, otherwise a
wrong icc_level will be calculated by zero "max_uA" and thus causes
unexpected results after it is written to device.

For regulators used by UFS, vcc, vccq and vccq2 will have voltage range
initialized by ufshcd_populate_vreg(), however other regulators may have
undefined voltage range if dt-bindings have no such definition.
In above undefined case, both "min_uV" and "max_uV" fields in ufs_vreg
struct will be zero values and these values will be configured on
regulators in different power modes.
Currently this may have no harm if both "min_uV" and "max_uV" always keep
"zero values" because regulator_set_voltage() will always bypass such
invalid values and return "good" results.
However improper values shall be fixed to avoid potential bugs. Simply
bypass voltage configuration if voltage range is not defined.
