2023-10-10scsi: qla2xxx: Remove unsupported ql2xenabledif optionManish Rangankar
[ Upstream commit e9105c4b7a9208a21a9bda133707624f12ddabc2 ] User accidently passed module parameter ql2xenabledif=1 which is unsupported. However, driver still initialized which lead to guard tag errors during device discovery. Remove unsupported ql2xenabledif=1 option and validate the user input. 2023-09-23scsi: qla2xxx: fix inconsistent TMF timeoutQuinn Tran
commit 009e7fe4a1ed52276b332842a6b6e23b07200f2d upstream. Different behavior were experienced of session being torn down vs not when TMF is timed out. When FW detects the time out, the session is torn down. When driver detects the time out, the session is not torn down. Allow TMF error to return to upper layer without session tear down. 2023-08-11scsi: qla2xxx: Wait for io return on terminate rportQuinn Tran
commit fc0cba0c7be8261a1625098bd1d695077ec621c9 upstream. System crash due to use after free. Current code allows terminate_rport_io to exit before making sure all IOs has returned. For FCP-2 device, IO's can hang on in HW because driver has not tear down the session in FW at first sign of cable pull. When dev_loss_tmo timer pops, terminate_rport_io is called and upper layer is about to free various resources. Terminate_rport_io trigger qla to do the final cleanup, but the cleanup might not be fast enough where it leave qla still holding on to the same resource. Wait for IO's to return to upper layer before resources are freed. 2023-03-11scsi: qla2xxx: Fix erroneous link downQuinn Tran
commit 3fbc74feb642deb688cc97f76d40b7287ddd4cb1 upstream. If after an adapter reset the appearance of link is not recovered, the devices are not rediscovered. This is result of a race condition between adapter reset (abort_isp) and the topology scan. During adapter reset, the ABORT_ISP_ACTIVE flag is set. Topology scan usually occurred after adapter reset. [ Upstream commit 26f9ce53817a8fd84b69a73473a7de852a24c897 ] Aborting commands that have already been sent to the firmware can cause BUG in qlt_free_cmd(): BUG_ON(cmd->sg_mapped) For instance: - Command passes rdx_to_xfer state, maps sgl, sends to the firmware - Reset occurs, qla2xxx performs ISP error recovery, aborts the command - Target stack calls qlt_abort_cmd() and then qlt_free_cmd() - BUG_ON(cmd->sg_mapped) in qlt_free_cmd() occurs because sgl was not unmapped Thus, unmap sgl in qlt_abort_cmd() for commands with the aborted flag set. 2022-04-15scsi: qla2xxx: Fix hang due to session stuckQuinn Tran
commit c02aada06d19a215c8291bd968a99a270e96f734 upstream. User experienced device lost. The log shows Get port data base command was queued up, failed, and requeued again. Every time it is requeued, it set the FCF_ASYNC_ACTIVE. This prevents any recovery code from occurring because driver thinks a recovery is in progress for this session. In essence, this session is hung. The reason it gets into this place is the session deletion got in front of this call due to link perturbation. Break the requeue cycle and exit. The session deletion code will trigger a session relogin. When a thread is in the process of re-establishing a session, a flag is set to prevent multiple threads / triggers from doing the same task. This flag was left on, where any attempt to relogin was locked out. Clear this flag, if the attempt has failed. An index of queue_pair_map also gets set to 0. qpair_id = find_first_zero_bit(ha->qpair_qid_map, ha->max_qpairs); set_bit(qpair_id, ha->qpair_qid_map); ha->queue_pair_map[qpair_id] = qpair; In the alloc_queue callback driver checks the map, if queue is already allocated: ha->queue_pair_map[qidx] This works fine as long as max_qpairs is greater than nvme_max_hw_queues(8) since the size of the queue_pair_map is equal to max_qpair. 2020-09-23scsi: qla2xxx: Move rport registration out of internal work_listQuinn Tran
commit cd4ed6b470f1569692b5d0d295b207f870570829 upstream. Currently, the rport registration is being called from a single work element that is used to process QLA internal "work_list". This work_list is meant for quick and simple task (ie no sleep). The Rport registration process sometime can be delayed by upper layer. This causes back pressure with the internal queue where other jobs are unable to move forward. This patch will schedule the registration process with a new work element (fc_port.reg_work). While the RPort is being registered, the current state of the fcport will not move forward until the registration is done. If the state of the fabric has changed, a new field/next_disc_state will record the next action on whether to 'DELETE' or 'Reverify the session/ADISC'. 2020-05-27scsi: qla2xxx: Fix hang when issuing nvme disconnect-all in NPIVArun Easi
[ Upstream commit 45a76264c26fd8cfd0c9746196892d9b7e2657ee ] In NPIV environment, a NPIV host may use a queue pair created by base host or other NPIVs, so the check for a queue pair created by this NPIV is not correct, and can cause an abort to fail, which in turn means the NVME command not returned. This leads to hang in nvme_fc layer in nvme_fc_delete_association() which waits for all I/Os to be returned, which is seen as hang in the application. commit 5a263892d7d0b4fe351363f8d1a14c6a75955475 upstream. qlt_free_session_done() tries to post async PRLO / LOGO, and waits for the completion of these async commands. If UNLOADING is set, this is doomed to timeout, because the async logout command will never complete. The only way to avoid waiting pointlessly is to fail posting these commands in the first place if the driver is in UNLOADING state. In general, posting any command should be avoided when the driver is UNLOADING. With this patch, "rmmod qla2xxx" completes without noticeable delay. It makes sense to set this flag before starting session teardown. Furthermore, use atomic test_and_set_bit() to avoid the shutdown being run multiple times in parallel. In qla2x00_disable_board_on_pci_error(), the test for UNLOADING is postponed until after the check for an already disabled PCI board. commit 8c5c660529209a0e324c1c1a35ce3f83d67a2aa5 upstream. The original patch was to resolve the lldd being able to be unloaded while being used to talk to the boot device of the system. However, the end result of the original patch is that any driver unload while a nvme controller is live via the lldd is now being prohibited. Given the module reference, the module teardown routine can't be called, thus there's no way, other than manual actions to terminate the controllers. 2020-02-11scsi: qla2xxx: Fix unbound NVME response lengthArun Easi

This is indicated by some combination of state flags. There was an instance when a high, and incorrect, response length was indicated causing driver to overrun buffers. Fix this by checking and limiting the response payload length.
