subsequent fix to it:
cc22522fd55e ("PCI: acpiphp: Use pci_assign_unassigned_bridge_resources() only for non-root bus")
40613da52b13 fixed a problem where hot-adding a device with large BARs
failed if the bridge windows programmed by firmware were not large enough.
cc22522fd55e ("PCI: acpiphp: Use pci_assign_unassigned_bridge_resources()
only for non-root bus") fixed a problem with 40613da52b13: an ACPI hot-add
of a device on a PCI root bus (common in the virt world) or firmware
sending ACPI Bus Check to non-existent Root Ports (e.g., on Dell Inspiron
7352/0W6WV0) caused a NULL pointer dereference and suspend/resume hangs.
Unfortunately the combination of 40613da52b13 and cc22522fd55e caused other
problems:
- Fiona reported that hot-add of SCSI disks in QEMU virtual machine fails
sometimes.
- Dongli reported a similar problem with hot-add of SCSI disks.
- Jonathan reported a console freeze during boot on bare metal due to an
error in radeon GPU initialization.
Revert both patches to avoid adding these problems. This means we will
again see the problems with hot-adding devices with large BARs and the NULL
pointer dereferences and suspend/resume issues that 40613da52b13 and
cc22522fd55e were intended to fix.

40613da52b13 ("PCI: acpiphp: Reassign resources on bridge if necessary")
changed acpiphp hotplug to use pci_assign_unassigned_bridge_resources()
which depends on bridge being available, however enable_slot() can be
called without bridge associated:
1. Legitimate case of hotplug on root bus (widely used in virt world)
2. A (misbehaving) firmware, that sends ACPI Bus Check notifications to
non existing root ports (Dell Inspiron 7352/0W6WV0), which end up at
enable_slot(..., bridge = 0) where bus has no bridge assigned to it.
acpihp doesn't know that it's a bridge, and bus specific 'PCI
subsystem' can't augment ACPI context with bridge information since
the PCI device to get this data from is/was not available.
Issue is easy to reproduce with QEMU's 'pc' machine, which supports PCI
hotplug on hostbridge slots. To reproduce, boot kernel at commit
40613da52b13 in VM started with following CLI (assuming guest root fs is
installed on sda1 partition):

The issue was discovered on Dell Inspiron 7352/0W6WV0 laptop with following
sequence:
1. Suspend to RAM
2. Wake up with the same backtrace being observed:
3. 2nd suspend to RAM attempt makes laptop freeze
Fix it by using __pci_bus_assign_resources() instead of
pci_assign_unassigned_bridge_resources() as we used to do, but only in case
when bus doesn't have a bridge associated (to cover for the case of ACPI
event on hostbridge or non existing root port).
That lets us keep hotplug on root bus working like it used to and at the
same time keeps resource reassignment usable on root ports (and other 1st
level bridges) that was fixed by 40613da52b13.

When using ACPI PCI hotplug, hotplugging a device with large BARs may fail
if bridge windows programmed by firmware are not large enough.

If a PCIe hotplug slot has an Attention Button, the normal hot-add flow is:
- Slot is empty and slot power is off
- User inserts card in slot and presses Attention Button
- OS blinks Power Indicator for 5 seconds
- After 5 seconds, OS turns on Power Indicator, turns on slot power, and
enumerates the device
Previously, if a user pressed the Attention Button on an *empty* slot,
pciehp logged the following messages and blinked the Power Indicator
until a second button press:
[0.000] pciehp: Button press: will power on in 5 sec
[0.001] # Power Indicator starts blinking
[5.001] # 5 second timeout; slot is empty, so we should cancel the
request to power on and turn off Power Indicator
[7.000] # Power Indicator still blinking
[8.000] # possible card insertion
[9.000] pciehp: Button press: canceling request to power on
The first button press incorrectly left the slot in BLINKINGON_STATE, so
the second was interpreted as a "cancel power on" event regardless of
whether a card was present.
If the slot is empty, turn off the Power Indicator and return from
BLINKINGON_STATE to OFF_STATE after 5 seconds, effectively canceling the
request to power on. Putting the slot in OFF_STATE also means the second
button press will correctly request a slot power on if the slot is
occupied.

In 2013, commits
2e35afaefe64 ("PCI: pciehp: Add reset_slot() method")
608c388122c7 ("PCI: Add slot reset option to pci_dev_reset()")
amended PCIe hotplug to mask Presence Detect Changed events during a
Secondary Bus Reset. The reset thus no longer causes gratuitous slot
bringdown and bringup.
However the commits neglected to serialize reset with code paths reading
slot registers. For instance, a slot bringup due to an earlier hotplug
event may see the Presence Detect State bit cleared during a concurrent
Secondary Bus Reset.
In 2018, commit
5b3f7b7d062b ("PCI: pciehp: Avoid slot access during reset")
retrofitted the missing locking. It introduced a reset_lock which
serializes a Secondary Bus Reset with other parts of pciehp.
Unfortunately the locking turns out to be overzealous: reset_lock is
held for the entire enumeration and de-enumeration of hotplugged devices,
including driver binding and unbinding.
Driver binding and unbinding acquires device_lock while the reset_lock
of the ancestral hotplug port is held. A concurrent Secondary Bus Reset
acquires the ancestral reset_lock while already holding the device_lock.
The asymmetric locking order in the two code paths can lead to AB-BA
deadlocks.

Fix by releasing the reset_lock during driver binding and unbinding,
thereby splitting and shrinking the critical section.
Driver binding and unbinding is protected by the device_lock() and thus
serialized with a Secondary Bus Reset. There's no need to additionally
protect it with the reset_lock. However, pciehp does not bind and
unbind devices directly, but rather invokes PCI core functions which
also perform certain enumeration and de-enumeration steps.
The reset_lock's purpose is to protect slot registers, not enumeration
and de-enumeration of hotplugged devices. That would arguably be the
job of the PCI core, not the PCIe hotplug driver. After all, an
AER-induced Secondary Bus Reset may as well happen during boot-time
enumeration of the PCI hierarchy and there's no locking to prevent that
either.
Exempting *de-enumeration* from the reset_lock is relatively harmless:
A concurrent Secondary Bus Reset may foil config space accesses such as
PME interrupt disablement. But if the device is physically gone, those
accesses are pointless anyway. If the device is physically present and
only logically removed through an Attention Button press or the sysfs
"power" attribute, PME interrupts as well as DMA cannot come through
because pciehp_unconfigure_device() disables INTx and Bus Master bits.
That's still protected by the reset_lock in the present commit.
Exempting *enumeration* from the reset_lock also has limited impact:
The exempted call to pci_bus_add_device() may perform device accesses
through pcibios_bus_add_device() and pci_fixup_device() which are now
no longer protected from a concurrent Secondary Bus Reset. Otherwise
there should be no impact.
In essence, the present commit seeks to fix the AB-BA deadlocks while
still retaining a best-effort reset protection for enumeration and
de-enumeration of hotplugged devices -- until a general solution is
implemented in the PCI core.

Stuart Hayes reports that an error handled by DPC at a Root Port results
in pciehp gratuitously bringing down a subordinate hotplug port:
RP -- UP -- DP -- UP -- DP (hotplug) -- EP
pciehp brings the slot down because the Link to the Endpoint goes down.
That is caused by a Hot Reset being propagated as a result of DPC.
Per PCIe Base Spec 5.0, section 6.6.1 "Conventional Reset":
For a Switch, the following must cause a hot reset to be sent on all
Downstream Ports: [...]
* The Data Link Layer of the Upstream Port reporting DL_Down status.
In Switches that support Link speeds greater than 5.0 GT/s, the
Upstream Port must direct the LTSSM of each Downstream Port to the
Hot Reset state, but not hold the LTSSMs in that state. This permits
each Downstream Port to begin Link training immediately after its
hot reset completes. This behavior is recommended for all Switches.
* Receiving a hot reset on the Upstream Port.
Once DPC recovers, pcie_do_recovery() walks down the hierarchy and
invokes pcie_portdrv_slot_reset() to restore each port's config space.
At that point, a hotplug interrupt is signaled per PCIe Base Spec r5.0,
section 6.7.3.4 "Software Notification of Hot-Plug Events":
If the Port is enabled for edge-triggered interrupt signaling using
MSI or MSI-X, an interrupt message must be sent every time the logical
AND of the following conditions transitions from FALSE to TRUE: [...]
* The Hot-Plug Interrupt Enable bit in the Slot Control register is
set to 1b.
* At least one hot-plug event status bit in the Slot Status register
and its associated enable bit in the Slot Control register are both
set to 1b.
Prevent pciehp from gratuitously bringing down the slot by clearing the
error-induced Data Link Layer State Changed event before restoring
config space. Afterwards, check whether the link has unexpectedly
failed to retrain and synthesize a DLLSC event if so.
Allow each pcie_port_service_driver (one of them being pciehp) to define
a slot_reset callback and re-use the existing pm_iter() function to
iterate over the callbacks.
Thereby, the Endpoint driver remains bound throughout error recovery and
may restore the device to working state.
Surprise removal during error recovery is detected through a Presence
Detect Changed event. The hotplug port is expected to not signal that
event as a result of a Hot Reset.
The issue isn't DPC-specific, it also occurs when an error is handled by
AER through aer_root_reset(). So while the issue was noticed only now,
it's been around since 2006 when AER support was first introduced.

The Qualcomm PCI bridge device (Device ID 0x0110) found in chipsets such as
SM8450 does not set the Command Completed bit unless writes to the Slot
Command register change "Control" bits.
This results in timeouts like below:

Writes to a Downstream Port's Slot Control register are PCIe hotplug
"commands." If the Port supports Command Completed events, software must
wait for a command to complete before writing to Slot Control again.
pcie_do_write_cmd() sets ctrl->cmd_busy when it writes to Slot Control. If
software notification is enabled, i.e., PCI_EXP_SLTCTL_HPIE and
PCI_EXP_SLTCTL_CCIE are set, ctrl->cmd_busy is cleared by pciehp_isr().
But when software notification is disabled, as it is when pcie_init()
powers off an empty slot, pcie_wait_cmd() uses pcie_poll_cmd() to poll for
command completion, and it neglects to clear ctrl->cmd_busy, which leads to
spurious timeouts:

The Power Fault Detected bit in the Slot Status register differs from
all other hotplug events in that it is sticky: It can only be cleared
after turning off slot power. Per PCIe r5.0, sec. 6.7.1.8:
If a power controller detects a main power fault on the hot-plug slot,
it must automatically set its internal main power fault latch [...].
The main power fault latch is cleared when software turns off power to
the hot-plug slot.
The stickiness used to cause interrupt storms and infinite loops which
were fixed in 2009 by commits 5651c48cfafe ("PCI pciehp: fix power fault
interrupt storm problem") and 99f0169c17f3 ("PCI: pciehp: enable
software notification on empty slots").
Unfortunately in 2020 the infinite loop issue was inadvertently
reintroduced by commit 8edf5332c393 ("PCI: pciehp: Fix MSI interrupt
race"): The hardirq handler pciehp_isr() clears the PFD bit until
pciehp's power_fault_detected flag is set. That happens in the IRQ
thread pciehp_ist(), which never learns of the event because the hardirq
handler is stuck in an infinite loop. Fix by setting the
power_fault_detected flag already in the hardirq handler.

ctrl->reset_lock rw-sem, passing the number of PCIe hotplug controllers in

This lockdep warning is triggered because with Thunderbolt, hotplug ports
are nested. When removing multiple devices in a daisy-chain, each hotplug
port's reset_lock may be acquired recursively. It's never the same lock, so
the lockdep splat is a false positive.
Because locks at the same hierarchy level are never acquired recursively, a
per-level lockdep class is sufficient to fix the lockdep warning.
The choice to use one lockdep subclass per pcie-hotplug controller in the
path to the root-bus was made to conserve class keys because their number
is limited and the complexity grows quadratically with number of keys
according to Documentation/locking/lockdep-design.rst.

Since commit 2a671f77ee49 ("s390/pci: fix use after free of zpci_dev")
the reference count of a zpci_dev is incremented between
pcibios_add_device() and pcibios_release_device() which was supposed to
prevent the zpci_dev from being freed while the common PCI code has
access to it. It was missed however that the handling of zPCI
availability events assumed that once zpci_zdev_put() was called no
later availability event would still see the device. With the previously
mentioned commit however this assumption no longer holds and we must
make sure that we only drop the initial long-lived reference the zPCI
subsystem holds exactly once.
Do so by introducing a zpci_device_reserved() function that handles when
a device is reserved. Here we make sure the zpci_dev will not be
considered for further events by removing it from the zpci_list.
This also means that the device actually stays in the
ZPCI_FN_STATE_RESERVED state between the time we know it has been
reserved and the final reference going away. We thus need to consider it
a real state instead of just a conceptual state after the removal. The
final cleanup of PCI resources, removal from zbus, and destruction of
the IOMMU stays in zpci_release_device() to make sure holders of the
reference do see valid data until the release.

Downstream Port Containment (PCIe r5.0, sec. 6.2.10) disables the link upon
an error and attempts to re-enable it when instructed by the DPC driver.
A slot which is both DPC- and hotplug-capable is currently powered off by
pciehp once DPC is triggered (due to the link change) and powered back up
on successful recovery. That's undesirable, the slot should remain powered
so the hotplugged device remains bound to its driver. DPC notifies the
driver of the error and of successful recovery in pcie_do_recovery() and
the driver may then restore the device to working state.
Moreover, Sinan points out that turning off slot power by pciehp may foil
recovery by DPC: Power off/on is a cold reset concurrently to DPC's warm
reset. Sathyanarayanan reports extended delays or failure in link
retraining by DPC if pciehp brings down the slot.
Fix by detecting whether a Link Down event is caused by DPC and awaiting
recovery if so. On successful recovery, ignore both the Link Down and the
subsequent Link Up event.
Afterwards, check whether the link is down to detect surprise-removal or
another DPC event immediately after DPC recovery. Ensure that the
corresponding DLLSC event is not ignored by synthesizing it and invoking
irq_wake_thread() to trigger a re-run of pciehp_ist().
The IRQ threads of the hotplug and DPC drivers, pciehp_ist() and
dpc_handler(), race against each other. If pciehp is faster than DPC, it
will wait until DPC recovery completes.
Recovery consists of two steps: The first step (waiting for link
disablement) is recognizable by pciehp through a set DPC Trigger Status
bit. The second step (waiting for link retraining) is recognizable through
a newly introduced PCI_DPC_RECOVERING flag.
If DPC is faster than pciehp, neither of the two flags will be set and
pciehp may glean the recovery status from the new PCI_DPC_RECOVERED flag.
The flag is zero if DPC didn't occur at all, hence DLLSC events are not
ignored by default.
pciehp waits up to 4 seconds before assuming that DPC recovery failed and
bringing down the slot. This timeout is not taken from the spec (it
doesn't mandate one) but based on a report from Yicong Yang that DPC may
take a bit more than 3 seconds on HiSilicon's Kunpeng platform.
The timeout is necessary because the DPC Trigger Status bit may never
clear: On Root Ports which support RP Extensions for DPC, the DPC driver
polls the DPC RP Busy bit for up to 1 second before giving up on DPC
recovery. Without the timeout, pciehp would then wait indefinitely for DPC
to complete.

The sysfs_emit() and sysfs_emit_at() functions were introduced to make
it less ambiguous which function is preferred when writing to the output
buffer in a device attribute's "show" callback [1].
Convert the PCI sysfs object "show" functions from sprintf(), snprintf()
and scnprintf() to sysfs_emit() and sysfs_emit_at() accordingly, as the
latter is aware of the PAGE_SIZE buffer and correctly returns the number
of bytes written into the buffer.
No functional change intended.

With zpci_configure_device() now always called on a device that has
already been configured on the platform level its name has become
misleading. Rename it to zpci_scan_configured_device() to signify that
the function now only handles the correct scanning of a newly configured
PCI function taking care of the special handling necessary for function
0 and functions parked waiting for a PCI bus that can't be created
without first seeing function 0.

Currently zpci_configure_device() can be called on a zPCI function in
two completely different states. Either the underlying zPCI function has
already been configured by the platform and we are only doing the
scanning to get it usable by Linux drivers. Or the underlying function
is in Standby and we first do an SCLP to get it configured. This makes
zpci_configure_device() harder to reason about. Since calling
zpci_configure_device() on a function in Standby only happens in
enable_slot() simply pull out the SCLP call and setting of zdev->state
and thus call zpci_configure_device() under the same circumstances as
in the event handling code.

In enable_slot(), if pci_get_slot() returns NULL, we clear the SLOT_ENABLED
flag. When pci_get_slot() finds a device, it increments the device's
reference count. In this case, we did not call pci_dev_put() to decrement
the reference count, so the memory of the device (struct pci_dev type) will
eventually leak.
Call pci_dev_put() to decrement its reference count when pci_get_slot()
returns a PCI device.

A zPCI event with PEC 0x0301 for an existing zPCI device goes through
the same actions as enable_slot(). Similarly a zPCI event with PEC
0x0303 does the same steps as disable_slot().
We can thus unify both actions as zpci_configure_device() respectively
zpci_deconfigure_device().

The current zdev->state mixes the configuration states supported by CLP
with an additional Online state which is used inconsistently to include
enabled zPCI functions which are not yet visible to the common PCI
subsytem. In preparation for a clean separation between architected
configuration states and fine grained function states remove the Online
function state.
Where we previously checked for Online it is more accurate to check if
the function is enabled to avoid an edge case where a disabled device
was still treated as Online. This also simplifies checks whether
a function is configured as this is now directly reflected by its
function state.

Both add_slot_store() and remove_slot_store() try to fix up the
drc_name copied from the store buffer by placing a NUL terminator at
nbyte + 1 or in place of a '\n' if present. However, the static buffer
that we copy the drc_name data into is not zeroed and can contain
anything past the n-th byte.
This is problematic if a '\n' byte appears in that buffer after nbytes
and the string copied into the store buffer was not NUL terminated to
start with as the strchr() search for a '\n' byte will mark this
incorrectly as the end of the drc_name string resulting in a drc_name
string that contains garbage data after the n-th byte.
Additionally it will cause us to overwrite that '\n' byte on the stack
with NUL, potentially corrupting data on the stack.
The following debugging shows an example of the drmgr utility writing
"PHB 4543" to the add_slot sysfs attribute, but add_slot_store()
logging a corrupted string value.

Fix this by using strscpy() instead of memcpy() to ensure the string
is NUL terminated when copied into the static drc_name buffer.
Further, since the string is now NUL terminated the code only needs to
change '\n' to '\0' when present.

In commit 05bc1be6db4b2 ("s390/pci: create zPCI bus") we removed the
pci_dev_put() call matching the earlier pci_get_slot() done as part of
__zpci_event_availability(). This was based on the wrong understanding
that the device_put() done as part of pci_destroy_device() would counter
the pci_get_slot() when it only counters the initial reference. This
same understanding and existing bad example also lead to not doing
a pci_dev_put() in zpci_remove_device().
Since releasing the PCI devices, unlike releasing the PCI slot, does not
print any debug message for testing I added one in pci_release_dev().
This revealed that we are indeed leaking the PCI device on PCI
hotunplug. Further testing also revealed another missing pci_dev_put() in
disable_slot().
Fix this by adding the missing pci_dev_put() in disable_slot() and fix
zpci_remove_device() with the correct pci_dev_put() calls. Also instead
of calling pci_get_slot() in __zpci_event_availability() to determine if
a PCI device is registered and then doing the same again in
zpci_remove_device() do this once in zpci_remove_device() which makes
sure that the pdev in __zpci_event_availability() is only used for the
result of pci_scan_single_device() which does not need a reference count
decremnt as its ownership goes to the PCI bus.
Also move the check if zdev->zbus->bus is set into zpci_remove_device()
since it may be that we're removing a device with devfn != 0 which never
had a PCI bus. So we can still set the pdev->error_state to indicate
that the device is not usable anymore, add a flag to set the error state.

When a PCIe card is hot-removed, the Presence Detect State and Data Link
Layer Link Active bits often do not clear simultaneously. I've seen delays
of up to 244 msec between the two events with Thunderbolt.
After pciehp has brought down the slot in response to the first event, the
other bit may still be set. It's not discernible whether it's set because
a new card is already in the slot or if it will soon clear. So pciehp
tries to bring up the slot and in the latter case fails with a bunch of
messages, some of them at KERN_ERR severity. If the slot is no longer
occupied, the messages are false positives and annoy users.
Stuart Hayes reports the following splat on hot removal:

Users are particularly irritated to see a bringup attempt even though the
slot was explicitly brought down via sysfs. In a perfect world, we could
avoid this by setting Link Disable on slot bringdown and re-enabling it
upon a Presence Detect State change. In reality however, there are broken
hotplug ports which hardwire Presence Detect to zero, see 80696f991424
("PCI: pciehp: Tolerate Presence Detect hardwired to zero"). Conversely,
PCIe r1.0 hotplug ports hardwire Link Active to zero because Link Active
Reporting wasn't specified before PCIe r1.1. On unplug, some ports first
clear Presence then Link (see Stuart Hayes' splat) whereas others use the
inverse order (see Dongdong Liu's splat). To top it off, there are hotplug
ports which flap the Presence and Link bits on slot bringup, see
6c35a1ac3da6 ("PCI: pciehp: Tolerate initially unstable link").
pciehp is designed to work with all of these variants. Surplus attempts at
slot bringup are a lesser evil than not being able to bring up slots at
all. Although we could try to perfect the behavior for specific hotplug
controllers, we'd risk breaking others or increasing code complexity.
But we can certainly minimize annoyance by emitting only a single message
with KERN_INFO severity if bringup is unsuccessful:
* Drop the "Timeout waiting for Presence Detect" message in
pcie_wait_for_presence(). The sole caller of that function,
pciehp_check_link_status(), ignores the timeout and carries on. It emits
error messages of its own and I don't think this particular message adds
much value.
* There's a single error condition in pciehp_check_link_status() which
does not emit a message. Adding one allows dropping the "Failed to check
link status" message emitted by board_added() if
pciehp_check_link_status() returns a non-zero integer.
* Tone down all messages in pciehp_check_link_status() to KERN_INFO
severity and rephrase them to look as innocuous as possible. To this
end, move the message emitted by pcie_wait_for_link_delay() to its
callers.
As a result, Stuart Hayes' splat becomes:

Replace the existing /* fall through */ comments and its variants with
the new pseudo-keyword macro fallthrough[1]. Also, remove unnecessary
fall-through markings when it is the case.

For fixing the PF to VF link removal we need to perform some action on
every removal of a zdev from the common PCI subsystem.
So in preparation re-introduce zpci_remove_device() and use that instead
of directly calling the common code functions. This was actually still
declared from earlier code but no longer implemented.

If context is not NULL in acpiphp_grab_context(), but the
is_going_away flag is set for the device's parent, the reference
counter of the context needs to be decremented before returning
NULL or the context will never be freed, so make that happen.

Since commit 84af7a6194e4 ("checkpatch: kconfig: prefer 'help' over
'---help---'"), the number of '---help---' has been gradually
decreasing, but there are still more than 2400 instances.
This commit finishes the conversion. While I touched the lines,
I also fixed the indentation.
There are a variety of indentation styles found.
a) 4 spaces + '---help---'
b) 7 spaces + '---help---'
c) 8 spaces + '---help---'
d) 1 space + 1 tab + '---help---'
e) 1 tab + '---help---' (correct indentation)
f) 1 tab + 1 space + '---help---'
g) 1 tab + 2 spaces + '---help---'
In order to convert all of them to 1 tab + 'help', I ran the
following commend:
$ find . -name 'Kconfig*' | xargs sed -i 's/^[[:space:]]*---help---/\thelp/'
