2021-03-11nvme: register ns_id attributes as default sysfs groupsHannes Reinecke
commit 33b14f67a4e1eabd219fd6543da8f15ed86b641c upstream. We should be registering the ns_id attribute as default sysfs attribute groups, otherwise we have a race condition between the uevent and the attributes appearing in sysfs. [Backport Notes] Resolve two context conflicts introduced by following two commits. These two commits are applied after the current commit in upstream, while have been merged into 4.19.y stable tree before the current commit. 1. drivers/nvme/host/core.c:nvme_ns_remove, introduced by commit 2181e455612a ("nvme: fix possible io failures when removing multipathed ns"). 2. drivers/nvme/host/multipath.c:nvme_mpath_set_live, introduced by commit 5e416b11b4a9 ("nvme-multipath: fix possible I/O hang when paths are updated"). 2021-03-11block: genhd: add 'groups' argument to device_add_diskHannes Reinecke

This avoids race condition the driver would otherwise have if these groups were to be created with sysfs_add_groups(). 2021-02-10nvme-pci: avoid the deepest sleep state on Kingston A2000 SSDsThorsten Leemhuis
commit 538e4a8c571efdf131834431e0c14808bcfb1004 upstream. Some Kingston A2000 NVMe SSDs sooner or later get confused and stop working when they use the deepest APST sleep while running Linux. The system then crashes and one has to cold boot it to get the SSD working again. Kingston seems to known about this since at least mid-September 2020: https://bbs.archlinux.org/viewtopic.php?pid=1926994#p1926994 Someone working for a German company representing Kingston to the German press confirmed to me Kingston engineering is aware of the issue and investigating; the person stated that to their current knowledge only the deepest APST sleep state causes trouble. Therefore, make Linux avoid it for now by applying the NVME_QUIRK_NO_DEEPEST_PS to this SSD. I have two such SSDs, but it seems the problem doesn't occur with them. I hence couldn't verify if this patch really fixes the problem, but all the data in front of me suggests it should. This patch can easily be reverted or improved upon if a better solution surfaces. This patch fixes mismatch among dbbuf for sq/cqs in case that dbbuf command fails. 2020-11-05nvme-rdma: fix crash when connect rejectedChao Leng
[ Upstream commit 43efdb8e870ee0f58633fd579aa5b5185bf5d39e ] A crash can happened when a connect is rejected. The host establishes the connection after received ConnectReply, and then continues to send the fabrics Connect command. If the controller does not receive the ReadyToUse capsule, host may receive a ConnectReject reply. Call nvme_rdma_destroy_queue_ib after the host received the RDMA_CM_EVENT_REJECTED event. Then when the fabrics Connect command times out, nvme_rdma_timeout calls nvme_rdma_complete_rq to fail the request. A crash happenes due to use after free in nvme_rdma_complete_rq. nvme_rdma_destroy_queue_ib is redundant when handling the RDMA_CM_EVENT_REJECTED event as nvme_rdma_destroy_queue_ib is already called in connection failure handler. 2020-10-07nvme-fc: fail new connections to a deleted host or remote portJames Smart
[ Upstream commit 9e0e8dac985d4bd07d9e62922b9d189d3ca2fccf ] The lldd may have made calls to delete a remote port or local port and the delete is in progress when the cli then attempts to create a new controller. Currently, this proceeds without error although it can't be very successful. Fix this by validating that both the host port and remote port are present when a new controller is to be created. [ Upstream commit 05b29021fba5e725dd385151ef00b6340229b500 ] Commit 3b4b19721ec652 ("nvme: fix possible deadlock when I/O is blocked") reverted multipath head disk revalidation due to deadlocks caused by holding the bd_mutex during revalidate. Updating the multipath disk blockdev size is still required though for userspace to be able to observe any resizing while the device is mounted. Directly update the bdev inode size to avoid unnecessarily holding the bdev->bd_mutex. [ Upstream commit 3b4b19721ec652ad2c4fe51dfbe5124212b5f581 ] Revert fab7772bfbcf ("nvme-multipath: revalidate nvme_ns_head gendisk in nvme_validate_ns") When adding a new namespace to the head disk (via nvme_mpath_set_live) we will see partition scan which triggers I/O on the mpath device node. This process will usually be triggered from the scan_work which holds the scan_lock. If I/O blocks (if we got ana change currently have only available paths but none are accessible) this can deadlock on the head disk bd_mutex as both partition scan I/O takes it, and head disk revalidation takes it to check for resize (also triggered from scan_work on a different path). The mpath disk revalidation was originally added to detect online disk size change, but this is no longer needed since commit cb224c3af4df ("nvme: Convert to use set_capacity_revalidate_and_notify") which already updates resize info without unnecessarily revalidating the disk (the mpath disk doesn't even implement .revalidate_disk fop). 2020-10-01nvmet-rdma: fix double free of rdma queueIsrael Rukshin
[ Upstream commit 21f9024355e58772ec5d7fc3534aa5e29d72a8b6 ] In case rdma accept fails at nvmet_rdma_queue_connect(), release work is scheduled. Later on, a new RDMA CM event may arrive since we didn't destroy the cm-id and call nvmet_rdma_queue_connect_fail(), which schedule another release work. This will cause calling nvmet_rdma_free_queue twice. To fix this we implicitly destroy the cm_id with non-zero ret code, which guarantees that new rdma_cm events will not arrive afterwards. Also add a qp pointer to nvmet_rdma_queue structure, so we can use it when the cm_id pointer is NULL or was destroyed. 2020-10-01nvme: Fix controller creation races with teardown flowIsrael Rukshin
[ Upstream commit ce1518139e6976cf19c133b555083354fdb629b8 ] Calling nvme_sysfs_delete() when the controller is in the middle of creation may cause several bugs. If the controller is in NEW state we remove delete_controller file and don't delete the controller. The user will not be able to use nvme disconnect command on that controller again, although the controller may be active. Other bugs may happen if the controller is in the middle of create_ctrl callback and nvme_do_delete_ctrl() starts. For example, freeing I/O tagset at nvme_do_delete_ctrl() before it was allocated at create_ctrl callback. To fix all those races don't allow the user to delete the controller before it was fully created. 2020-10-01nvme-multipath: do not reset on unknown statusJohn Meneghini

There are, however, no existing nvme status codes that indicate a reset should be used, and resetting causes unnecessary disruption to the rest of IO. Change nvme's error handling to first check if failover should happen. If not, let the normal error handling take over rather than reset the controller. 2020-09-17nvme-rdma: serialize controller teardown sequencesSagi Grimberg
[ Upstream commit 5110f40241d08334375eb0495f174b1d2c07657e ] In the timeout handler we may need to complete a request because the request that timed out may be an I/O that is a part of a serial sequence of controller teardown or initialization. In order to complete the request, we need to fence any other context that may compete with us and complete the request that is timing out. In this case, we could have a potential double completion in case a hard-irq or a different competing context triggered error recovery and is running inflight request cancellation concurrently with the timeout handler. Protect using a ctrl teardown_lock to serialize contexts that may complete a cancelled request due to error recovery or a reset. [ Upstream commit 489dd102a2c7c94d783a35f9412eb085b8da1aa4 ] When scan_work calls nvme_mpath_add_disk() this holds ana_lock and invokes nvme_parse_ana_log(), which may issue IO in device_add_disk() and hang waiting for an accessible path. While nvme_mpath_set_live() only called when nvme_state_is_live(), a transition may cause NVME_SC_ANA_TRANSITION and requeue the IO. In order to recover and complete the IO ana_work on the same ctrl should be able to update the path state and remove NVME_NS_ANA_PENDING. The deadlock occurs because scan_work keeps holding ana_lock, so ana_work hangs [1]. Fix: Now nvme_mpath_add_disk() uses nvme_parse_ana_log() to obtain a copy of the ANA group desc, and then calls nvme_update_ns_ana_state() without holding ana_lock. [ Upstream commit 657f1975e9d9c880fa13030e88ba6cc84964f1db ] The deadlock combines 4 flows in parallel: - ns scanning (triggered from reconnect) - request timeout - ANA update (triggered from reconnect) - I/O coming into the mpath device (1) ns scanning triggers disk revalidation -> update disk info -> freeze queue -> but blocked, due to (2) (2) timeout handler reference the g_usage_counter - > but blocks in the transport .timeout() handler, due to (3) (3) the transport timeout handler (indirectly) calls nvme_stop_queue() -> which takes the (down_read) namespaces_rwsem - > but blocks, due to (4) (4) ANA update takes the (down_write) namespaces_rwsem -> calls nvme_mpath_set_live() -> which synchronize the ns_head srcu (see commit 504db087aacc) -> but blocks, due to (5) (5) I/O came into nvme_mpath_make_request -> took srcu_read_lock -> direct_make_request > blk_queue_enter -> but blocked, due to (1) ==> the request queue is under freeze -> deadlock. The fix is making ANA update take a read lock as the namespaces list is not manipulated, it is just the ns and ns->head that are being updated (which is protected with the ns->head lock). 2020-04-17nvme: Treat discovery subsystems as unique subsystemsJames Smart
commit c26aa572027d438de9cc311aaebcbe972f698c24 upstream. Current code matches subnqn and collapses all controllers to the same subnqn to a single subsystem structure. This is good for recognizing multiple controllers for the same subsystem. But with the well-known discovery subnqn, the subsystems aren't truly the same subsystem. As such, subsystem specific rules, such as no overlap of controller id, do not apply. With today's behavior, the check for overlap of controller id can fail, preventing the new discovery controller from being created. When searching for like subsystem nqn, exclude the discovery nqn from matching. This will result in each discovery controller being attached to a unique subsystem structure. [ Upstream commit 3a8ecc935efabdad106b5e06d07b150c394b4465 ] Commit 7fd8930f26be4 "nvme: add a common helper to read Identify Controller data" has re-introduced an issue that we have attempted to work around in the past, in commit a310acd7a7ea ("NVMe: use split lo_hi_{read,write}q"). The problem is that some PCIe NVMe controllers do not implement 64-bit outbound accesses correctly, which is why the commit above switched to using lo_hi_[read|write]q for all 64-bit BAR accesses occuring in the code. In the mean time, the NVMe subsystem has been refactored, and now calls into the PCIe support layer for NVMe via a .reg_read64() method, which fails to use lo_hi_readq(), and thus reintroduces the problem that the workaround above aimed to address. Given that, at the moment, .reg_read64() is only used to read the capability register [which is known to tolerate split reads], let's switch .reg_read64() to lo_hi_readq() as well. This fixes a boot issue on some ARM boxes with NVMe behind a Synopsys DesignWare PCIe host controller. 2020-01-09nvme-fc: fix double-free scenarios on hw queuesJames Smart
[ Upstream commit c869e494ef8b5846d9ba91f1e922c23cd444f0c1 ] If an error occurs on one of the ios used for creating an association, the creating routine has error paths that are invoked by the command failure and the error paths will free up the controller resources created to that point. But... the io was ultimately determined by an asynchronous completion routine that detected the error and which unconditionally invokes the error_recovery path which calls delete_association. Delete association deletes all outstanding io then tears down the controller resources. So the create_association thread can be running in parallel with the error_recovery thread. What was seen was the LLDD received a call to delete a queue, causing the LLDD to do a free of a resource, then the transport called the delete queue again causing the driver to repeat the free call. The second free routine corrupted the allocator. The transport shouldn't be making the duplicate call, and the delete queue is just one of the resources being freed. To fix, it is realized that the create_association path is completely serialized with one command at a time. So the failed io completion will always be seen by the create_association path and as of the failure, there are no ios to terminate and there is no reason to be manipulating queue freeze states, etc. The serialized condition stays true until the controller is transitioned to the LIVE state. Thus the fix is to change the error recovery path to check the controller state and only invoke the teardown path if not already in the CONNECTING state. [ Upstream commit 863fbae929c7a5b64e96b8a3ffb34a29eefb9f8f ] In nvme-fc: it's possible to have connected active controllers and as no references are taken on the LLDD, the LLDD can be unloaded. The controller would enter a reconnect state and as long as the LLDD resumed within the reconnect timeout, the controller would resume. But if a namespace on the controller is the root device, allowing the driver to unload can be problematic. To reload the driver, it may require new io to the boot device, and as it's no longer connected we get into a catch-22 that eventually fails, and the system locks up. Fix this issue by taking a module reference for every connected controller (which is what the core layer did to the transport module). Reference is cleared when the controller is removed. Two different variants of this behavior have been observed: SM22XX controllers round up the read size to a multiple of 512 bytes, and Phison E12 unconditionally reads the maximum discard size allowed by the spec (256 segments or 4kB). Make nvme_setup_discard unconditionally allocate the maximum DSM buffer so the driver DMA maps a memory range that will always succeed. If we fail allocating the special page, we return busy, and it'll get retried. But since ordering is honored for dispatch requests, we can keep retrying this same IO and failing. Behind that IO could be requests that want to free memory, but they never get the chance. Allocate a fixed discard page per controller for a safe fallback, and use that if the initial allocation fails. 2019-12-01nvme-pci: fix hot removal during error handlingKeith Busch
[ Upstream commit cb4bfda62afa25b4eee3d635d33fccdd9485dd7c ] A removal waits for the reset_work to complete. If a surprise removal occurs around the same time as an error triggered controller reset, and reset work happened to dispatch a command to the removed controller, the command won't be recovered since the timeout work doesn't do anything during error recovery. We wouldn't want to wait for timeout handling anyway, so this patch fixes this by disabling the controller and killing admin queues prior to syncing with the reset_work. 2019-11-12nvme-multipath: fix possible io hang after ctrl reconnectAnton Eidelman
[ Upstream commit af8fd0424713a2adb812d10d55e86718152cf656 ] The following scenario results in an IO hang: 1) ctrl completes a request with NVME_SC_ANA_TRANSITION. NVME_NS_ANA_PENDING bit in ns->flags is set and ana_work is triggered. 2) ana_work: nvme_read_ana_log() tries to get the ANA log page from the ctrl. This fails because ctrl disconnects. Therefore nvme_update_ns_ana_state() is not called and NVME_NS_ANA_PENDING bit in ns->flags is not cleared. 3) ctrl reconnects: nvme_mpath_init(ctrl,...) calls nvme_read_ana_log(ctrl, groups_only=true). However, nvme_update_ana_state() does not update namespaces because nr_nsids = 0 (due to groups_only mode). 4) scan_work calls nvme_validate_ns() finds the ns and re-validates OK. Result: The ctrl is now live but NVME_NS_ANA_PENDING bit in ns->flags is still set. Consequently ctrl will never be considered a viable path by __nvme_find_path(). IO will hang if ctrl is the only or the last path to the namespace. More generally, while ctrl is reconnecting, its ANA state may change. And because nvme_mpath_init() requests ANA log in groups_only mode, these changes are not propagated to the existing ctrl namespaces. This may result in a mal-function or an IO hang. Solution: nvme_mpath_init() will nvme_read_ana_log() with groups_only set to false. This will not harm the new ctrl case (no namespaces present), and will make sure the ANA state of namespaces gets updated after reconnect. Note: Another option would be for nvme_mpath_init() to invoke nvme_parse_ana_log(..., nvme_set_ns_ana_state) for each existing namespace. 2019-10-05nvme-multipath: fix ana log nsid lookup when nsid is not foundAnton Eidelman
[ Upstream commit e01f91dff91c7b16a6e3faf2565017d497a73f83 ] ANA log parsing invokes nvme_update_ana_state() per ANA group desc. This updates the state of namespaces with nsids in desc->nsids[]. Both ctrl->namespaces list and desc->nsids[] array are sorted by nsid. Hence nvme_update_ana_state() performs a single walk over ctrl->namespaces: - if current namespace matches the current desc->nsids[n], this namespace is updated, and n is incremented. - the process stops when it encounters the end of either ctrl->namespaces end or desc->nsids[] In case desc->nsids[n] does not match any of ctrl->namespaces, the remaining nsids following desc->nsids[n] will not be updated. Such situation was considered abnormal and generated WARN_ON_ONCE. However ANA log MAY contain nsids not (yet) found in ctrl->namespaces. For example, lets consider the following scenario: - nvme0 exposes namespaces with nsids = [2, 3] to the host - a new namespace nsid = 1 is added dynamically - also, a ANA topology change is triggered - NS_CHANGED aen is generated and triggers scan_work - before scan_work discovers nsid=1 and creates a namespace, a NOTICE_ANA aen was issues and ana_work receives ANA log with nsids=[1, 2, 3] Result: ana_work fails to update ANA state on existing namespaces [2, 3] Solution: Change the way nvme_update_ana_state() namespace list walk checks the current namespace against desc->nsids[n] as follows: a) ns->head->ns_id < desc->nsids[n]: keep walking ctrl->namespaces. b) ns->head->ns_id == desc->nsids[n]: match, update the namespace c) ns->head->ns_id >= desc->nsids[n]: skip to desc->nsids[n+1] This enables correct operation in the scenario described above. This also allows ANA log to contain nsids currently invisible to the host, i.e. inactive nsids.
