Add the missing field initializers for a couple of the TLV attribute
macros. This resolves the last few -Wmissing-field-initializers warnings
for the fm10k Linux driver.

After many years of having a ~30 line copyright and license header to our
source files, we are finally able to reduce that to one line with the
advent of the SPDX identifier.
Also caught a few files missing the SPDX license identifier, so fixed
them up.

Make functions that should be static. While we're at it, fix the function
header comment for fm10k_tlv_attr_nest_stop(), and update the copyright
header for fm10k_pf.h, fm10k_tlv.c and fm10k_tlv.h.

This patch adds generic mailbox support. The general idea of the mailboxes
is to use a pair of ring buffers, one for request, one for response to send
data between the local driver and some remote entity be it the PF of the
Switch Manager.
