Releasing the `priv->lock` while iterating the `priv->multicast_list` in
`ipoib_mcast_join_task()` opens a window for `ipoib_mcast_dev_flush()` to
remove the items while in the middle of iteration. If the mcast is removed
while the lock was dropped, the for loop spins forever resulting in a hard
lockup (as was reported on RHEL 4.18.0-372.75.1.el8_6 kernel):

Fix this by keeping the lock held and changing to GFP_ATOMIC to prevent
eventual sleeps.
Unfortunately we could not reproduce the lockup and confirm this fix but
based on the code review I think this fix should address such lockups.

The cited commit creates child PKEY interfaces over netlink will
multiple tx and rx queues, but some devices doesn't support more than 1
tx and 1 rx queues. This causes to a crash when traffic is sent over the
PKEY interface due to the parent having a single queue but the child
having multiple queues.
This patch fixes the number of queues to 1 for legacy IPoIB at the
earliest possible point in time.

There are 2 ways to create IPoIB PKEY child interfaces:
1) Writing a PKEY to /sys/class/net/<ib parent interface>/create_child.
2) Using netlink with iproute.
While with sysfs the child interface has the same number of tx and
rx queues as the parent, with netlink there will always be 1 tx
and 1 rx queue for the child interface. That's because the
get_num_tx/rx_queues() netlink ops are missing and the default value
of 1 is taken for the number of queues (in rtnl_create_link()).
This change adds the get_num_tx/rx_queues() ops which allows for
interfaces with multiple queues to be created over netlink. This
constant only represents the max number of tx and rx queues on that
net device.

After the commit 5ce2dced8e95 ("RDMA/ipoib: Set rtnl_link_ops for ipoib
interfaces"), if the IPoIB device is moved to non-initial netns,
destroying that netns lets the device vanish instead of moving it back to
the initial netns, This is happening because default_device_exit() skips
the interfaces due to having rtnl_link_ops set.

on shutdown due to a NULL pointer dereference, make sure to set the
netns_refund flag that was introduced by commit 3a5ca857079e ("can: dev:

Report the "ipoib pkey", "mode" and "umcast" netlink attributes for every
IPoiB interface type, not just children created with 'ip link add'.
After setting the rtnl_link_ops for the parent interface, implement the
dellink() callback to block users from trying to remove it.

ipoib_mcast_carrier_on_task() insanely open codes a rtnl_lock() such that
the only time flush_workqueue() can be called is if it also clears
IPOIB_FLAG_OPER_UP.
Thus the flush inside ipoib_flush_ah() will deadlock if it gets unlucky
enough, and lockdep doesn't help us to find it early:

Clean up the ah_reaper related functions and lifecycle to make sense:
- Start/Stop of the reaper should only be done in open/stop NDOs, not in
any other places
- cancel and flush of the reaper should only happen in the stop NDO.
cancel is only functional when combined with IPOIB_STOP_REAPER.
- Non-stop places were flushing the AH's just need to flush out dead AH's
synchronously and ignore the background task completely. It is fully
locked and harmless to leave running.
Which ultimately fixes the ABBA deadlock by removing the unnecessary
flush_workqueue() from the problematic place under the vlan_rwsem.

When connected mode is set, and we have connected and datagram traffic in
parallel, ipoib might crash with double free of datagram skb.
The current mechanism assumes that the order in the completion queue is
the same as the order of sent packets for all QPs. Order is kept only for
specific QP, in case of mixed UD and CM traffic we have few QPs (one UD and
few CM's) in parallel.

In this case (due to heavy load) UD2 skb pointer could be replaced by new
transmitted packet UD_NEW, as the driver assumes its free. At this point
we will have to process two 'wc' with same index but we have only one
pointer to free.
During second attempt to free the same skb we will have NULL pointer
exception.
2. UD2 'wc' processing
- skb freed according the index we got from 'wc', but it was already
overwritten by mistake. So actually the skb that was released is the
skb of the new transmitted packet and not the original one.
3. UD_NEW 'wc' processing
- attempt to free already freed skb. NUll pointer exception.
The fix:

The fix is to stop using the UD ring as a placeholder for CM packets, the
cyclic ring variables tx_head and tx_tail will manage the UD tx_ring, a
new cyclic variables global_tx_head and global_tx_tail are introduced for
managing and counting the overall outstanding sent packets, then the send
queue will be stopped and waken based on these variables only.
Note that no locking is needed since global_tx_head is updated in the xmit
flow and global_tx_tail is updated in the NAPI flow only. A previous
attempt tried to use one variable to count the outstanding sent packets,
but it did not work since xmit and NAPI flows can run at the same time and
the counter will be updated wrongly. Thus, we use the same simple cyclic
head and tail scheme that we have today for the UD tx_ring.

Despite failure in ipoib_dev_init() we continue with initialization flow
and creation of child device. It causes to the situation where this child
device is added too early to parent device list.
Change the logic, so in case of failure we properly return error from
ipoib_dev_init() and add child only in success path.

The InfiniBand docs are plain text with no markups. So, all we needed to
do were to add the title markups and some markup sequences in order to
properly parse tables, lists and literal blocks.
At its new index.rst, let's add a :orphan: while this is not linked to the
main index.rst file, in order to avoid build warnings.

The default action for unlisted tests is "not-supported", so given that
ipoib doesn't support ETH_SS_TEST, there is no need to check for it
in the case statements, just let it get caught by the default: case.

Systemd triggers the following warning during IPoIB device load:
mlx5_core 0000:00:0c.0 ib0: "systemd-udevd" wants to know my dev_id.
Should it look at dev_port instead?
See Documentation/ABI/testing/sysfs-class-net for more info.
This is caused due to user space attempt to differentiate old systems
without dev_port and new systems with dev_port. In case dev_port will be
zero, the systemd will try to read dev_id instead.
There is no need to print a warning in such case, because it is valid
situation and it is needed to ensure systemd compatibility with old
kernels.

When IPoIB receives an SM LID change event, it reacts by flushing its
path record cache and rejoining multicast groups. This is the same
behavior it performs when it receives a reregistration event. This
behavior is unnecessary as an SM may have database backup or
synchronization mechanisms which permit the SM location or LID to change
without loss of multicast membership and without impact to path records.
Both opensm and the OPA FM issue reregistration events if a new SM is
started (or restarted with a new config) or an SM event occurs which
results in loss of multicast membership records by the SM (such as
opensm failover) or the SM encounters new nodes with Active ports (such
as after joining 2 fabrics by connecting switches via ISLs). Hence this
event can be depended on as the trigger for IPoIB cache and multicast
flushing.
It appears that some drivers, such as qib, and hfi1 issue the
IB_EVENT_SM_CHANGE but other drivers such as mlx4 and mlx5 do not.
Empirical testing on Mellanox EDR using ibv_asyncwatch has confirmed
that Mellanox EDR HCAs do not generate SM change events and that opensm
does generate reregistration.
An SM LID change event is generated by the mentioned drivers to reflect
that sm_lid and/or sm_sl in the local port info has changed. The intent
of this event is to permit applications and ULPs which have a local copy
of this information (or an address handle using it) to update their
information.
The intent is that the reregistration event (caused by the SM via a bit
in Set(PortInfo)) be used to inform nodes that they need to rejoin
multicast groups, resubscribe for notices and potentially update path
records.
When an SM migrates or fails over, a SM LID change event can occur. In
response IPoIB discards path records and multicast membership and loses
connectivity until these records are restored via SA requests. In very
large fabrics, it may take minutes for the SM to be ready and for the SA
responses to be supplied. This can result in undesirable and
unnecessary IPoIB connectivity impacts. It also can result in an
unnecessary storm of SA queries from all nodes in a cluster potentially
followed by yet another storm if the SM issues the reregistration
request.
The fact the Mellanox HCAs do not even generate this event, is further
evidence that on modern IB fabrics there will be no ill side effects
from the proposed changes below to reduce the reaction by 3 kernel
components to this event. So these changes should be benign for Mellanox
IB fabrics and will benefit OPA fabrics while also making ib_core and
ULP behavor "correct" as intended by the IBTA spec and kernel RDMA event
APIs.
Address these issues by removing IB_EVENT_SM_CHANGE handling from ipoib.
IPoIB does not locally store sm_lid nor sm_sl, so it does not need to do
anything on SM LID change. IPoIB makes use of other ib_core components
to issue SA requests for it and those components correctly track SM LID
and SM LID changes.
Also in ib_core multicast handling, remove the test for
IB_EVENT_SM_CHANGE. This code is moving all multicast groups to the
error state, which will trigger rejoins. This code is used by IPoIB as
well as the connection manager and other clients of multicast groups.
This kernel module centralizes group membership status and joins since a
node can only join a given group once but multiple ULPs or applications
may want to join the same group. It makes use of the sa_query.c
component in ib_core, which correctly trackes SM LID and SL. This
component does not track SM LID nor SL itself and hence need not react
to their changes.
Similarly in the ib_core cache code remove the handling for the
IB_EVENT_SM_CHANGE. In this function. The ib_cache_update function
which is ultimately called is updating local copies of the pkey table,
gid table and lmc. It does not update nor retain sm_lid nor sm_sl. As
such it does not need to be called on an SM LID change. It technically
also does not need to be called on a reregistration. The LID_CHANGE,
PKEY_CHANGE, GID_CHANGE and port state change events (PORT_ERR,
PORT_ACTICE) should be sufficient triggers.
It is worth noting that the alternative of simply having the hfi1 and
qib drivers not generate the SM LID change event was explored. While
this would duplicate what Mellanox drivers do now, it is not the correct
behavior and removes the ability for an SM to migrate without requiring
reregistration. Since both opensm and OPA SM have mechanisms to backup
or synchronize registration information, it is desirable to let them
perform SM migrations (with LID or SL changes) without requiring
reregistration when they deem it appropriate.

In order to pass extack together with NETDEV_PRE_UP notifications, it's
necessary to route the extack to __dev_open() from diverse (possibly
indirect) callers. One prominent API through which the notification is
invoked is dev_change_flags().
Therefore extend dev_change_flags() with and extra extack argument and
update all users. Most of the calls end up just encoding NULL, but
several sites (VLAN, ipvlan, VRF, rtnetlink) do have extack available.
Since the function declaration line is changed anyway, name the other
function arguments to placate checkpatch.

IPoIB netlink support was broken by the below commit since integrating
the rdma_netdev support relies on an allocation flow for netdevs that
was controlled by the ipoib driver while netdev's rtnl_newlink
implementation assumes that the netdev will be allocated by netlink.
Such situation leads to crash in __ipoib_device_add, once trying to
reuse netlink device.
This patch fixes the kernel oops for both mlx4 and mlx5
devices triggered by the following command:

netdev has several interfaces that expect to call alloc_netdev_mqs from
the core code, with the driver only providing the arguments. This is
incompatible with the rdma_netdev interface that returns the netdev
directly.
Thus re-organize the API used by ipoib so that the verbs core code calls
alloc_netdev_mqs for the driver. This is done by allowing the drivers to
provide the allocation parameters via a 'get_params' callback and then
initializing an allocated netdev as a second step.

Some tools may currently be using only the deprecated attribute;
let's print an elaborate and clear deprecation notice to kmsg.
To do that, we have to replace the whole sysfs file, since we inherit
the original one from netdev.

Some InfiniBand network devices have multiple ports on the same PCI
function. This initializes the `dev_port' sysfs field of those
network interfaces with their port number.
Prior to this the kernel erroneously used the `dev_id' sysfs
field of those network interfaces to convey the port number to userspace.
The use of `dev_id' was considered correct until Linux 3.15,
when another field, `dev_port', was defined for this particular
purpose and `dev_id' was reserved for distinguishing stacked ifaces
(e.g: VLANs) with the same hardware address as their parent device.
Similar fixes to net/mlx4_en and many other drivers, which started
exporting this information through `dev_id' before 3.15, were accepted
into the kernel 4 years ago.

It is illegal to change MTU to a value lower than the minimum MTU
stated in ethernet spec. In addition to that we need to add 4 bytes
for encapsulation header (IPOIB_ENCAP_LEN).
Before "ifconfig ib0 mtu 0" command, succeeds while it obviously shouldn't.

Inside of start_xmit() the call to check if the connection is up and the
queueing of the packets for later transmission is not atomic which leaves
a window where cm_rep_handler can run, set the connection up, dequeue
pending packets and leave the subsequently queued packets by start_xmit()
sitting on neigh->queue until they're dropped when the connection is torn
down. This only applies to connected mode. These dropped packets can
really upset TCP, for example, and cause multi-minute delays in
transmission for open connections.
Here's the code in start_xmit where we check to see if the connection is
up:

The race occurs if cm_rep_handler execution occurs after the above
connection check (specifically if it gets to the point where it acquires
priv->lock to dequeue pending skb's) but before the below code snippet in
start_xmit where packets are queued.

The patch acquires the netif tx lock in cm_rep_handler for the section
where it sets the connection up and dequeues and retransmits deferred
skb's.

This fixes a bug in the netlink path where the vlan_rwsem was not
held around __ipoib_vlan_add causing the child_intfs to be manipulated
unsafely.
In the process this greatly simplifies the vlan_rwsem write side locking
to only cover a single non-sleeping statement.
This also further increases the safety of the removal ordering by holding
the netdev of the parent while the child is active to ensure most bugs
become either an oops on a NULL priv or a deadlock on the netdev refcount.

Switching to priv_destructor and needs_free_netdev created a subtle
ordering problem in ipoib_remove_one.
Now that unregister_netdev frees the netdev and priv we must ensure that
the children are unregistered before trying to unregister the parent,
or child unregister will use after free.
The solution is to unregister the children, then parent, in the same batch
all while holding the rtnl_lock. This closes all the races where a new
child could have been added and ensures proper ordering.

This mutex was introduced to deal with the deadlock formed by calling
unregister_netdev from within the sysfs callback of a netdev.
Now that we have priv_destructor and needs_free_netdev we can switch
to the more targeted solution of running the unregister from a
work queue. This avoids the deadlock and gets rid of the mutex.
The next patch in the series needs this mutex eliminated to create
atomicity of unregisteration.

Now that the unregister_netdev flow for IPoIB no longer relies on external
code we can now introduce the use of priv_destructor and
needs_free_netdev.
The rdma_netdev flow is switched to use the netdev common priv_destructor
instead of the special free_rdma_netdev and the IPOIB ULP adjusted:
- priv_destructor needs to switch to point to the ULP's destructor
which will then call the rdma_ndev's in the right order
- We need to be careful around the error unwind of register_netdev
as it sometimes calls priv_destructor on failure
- ULPs need to use ndo_init/uninit to ensure proper ordering
of failures around register_netdev
Switching to priv_destructor is a necessary pre-requisite to using
the rtnl new_link mechanism.
The VNIC user for rdma_netdev should also be revised, but that is left for
another patch.

Now that we have a proper ndo_uninit, move code that naturally pairs
with the ndo_uninit into ndo_init. This allows the netdev core to natually
handle ordering.
This fixes the situation where register_netdev can fail before calling
ndo_init, in which case it wouldn't call ndo_uninit either.
Also move a bunch of duplicated init code that is shared between child
and parent for clarity. Now the child and parent register functions look
very similar.
