There are 2 HISAX_AVM_A1_PCMCIA Kconfig entries. The kbuild system
ignores the second one, and apparently nobody noticed the problem so
far, so let's remove that second entry.

The first half of the HiSax config options is presented if
ISDN_DRV_HISAX!=n, while the second half of the options is presented
if ISDN_DRV_HISAX. That's the same, so merge both conditionals.

The HISAX_HFC4S8S_PCIMEM code hasn't been able to compile since before
the start of git history. I have deleted it.
There are also a few indenting mistakes where one side of the ifdef
wasn't indented correctly which I fixed as well.

The state machine code in the elsa driver uses interruptible_sleep_on
to wait for state changes, which is racy. A closer look at the possible
states reveals that it is always used to wait for getting back into
ARCOFI_NOP, so we can use wait_event_interruptible instead.

With arm:allmodconfig, building the Teles PCI driver fails with
telespci.c:294:2: error: #error "not running on big endian machines now"
Similar, building the driver for HFC PCI-Bus cards fails with
hfc_pci.c:1647:2: error: #error "not running on big endian machines now"
Remove the big endian cpp check from both drivers to fix the build errors.

Avoid unneeded local string buffers for constructing debug output. Also
cleans up debug calls that contain a single parameter so that they cannot
be accidentally parsed as format strings.

Disabling CONFIG_VIRT_TO_BUS on ARM showed that the hisax netjet
driver depends on this deprecated functionality but is not
marked so in Kconfig.
Rather than adding ARM to the already long list of architectures
that this driver is broken on, this patch adds 'depends on
VIRT_TO_BUS' and removes the dependency on !SPARC, which is
also implied by that.

The CONFIG_EXPERIMENTAL config item has not carried much meaning for a
while now and is almost always enabled by default. As agreed during the
Linux kernel summit, remove it from any "depends on" lines in Kconfigs.

CONFIG_HOTPLUG is going away as an option. As a result, the __dev*
markings need to be removed.
This change removes the use of __devinit, __devexit_p, __devinitdata,
and __devexit from these drivers.
Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.

Hub-initiated LPM is not good for USB communications devices. Comms
devices should be able to tell when their link can go into a lower power
state, because they know when an incoming transmission is finished.
Ideally, these devices would slam their links into a lower power state,
using the device-initiated LPM, after finishing the last packet of their
data transfer.
If we enable the idle timeouts for the parent hubs to enable
hub-initiated LPM, we will get a lot of useless LPM packets on the bus
as the devices reject LPM transitions when they're in the middle of
receiving data. Worse, some devices might blindly accept the
hub-initiated LPM and power down their radios while they're in the
middle of receiving a transmission.
The Intel Windows folks are disabling hub-initiated LPM for all USB
communications devices under a xHCI USB 3.0 host. In order to keep
the Linux behavior as close as possible to Windows, we need to do the
same in Linux.
Set the disable_hub_initiated_lpm flag for for all USB communications
drivers. I know there aren't currently any USB 3.0 devices that
implement these class specifications, but we should be ready if they do.

isdn source code uses a not-current coding style.
Update the coding style used on a per-line basis
so that git diff -w shows only elided blank lines
at EOF.
Done with emacs and some scripts and some typing.
Built x86 allyesconfig.

The file st5481_init.c locally defines and initializes the adapter_list
variable, but does not use it for anything. Removing the list makes it
possible to remove the list field from the st5481_adapter data structure.
In the function probe_st5481, it also makes it possible to free the locally
allocated adapter value on an error exit.
