This was caused by the stack trace code in save_stack_trace_tsk using
the wrong stack pointer. It was using the user stack pointer instead of
the kernel stack pointer. Fix this by using the right stack.
Also for good measure we add try_get_task_stack/put_task_stack to ensure
the task is not lost while we are walking it's stack.

Since v5.8-rc1 OpenRISC Linux fails to boot when DEBUG_VM is enabled.
This has been bisected to commit 42fc541404f2 ("mmap locking API: add
mmap_assert_locked() and mmap_assert_write_locked()").
The added locking checks exposed the issue that OpenRISC was not taking
this mmap lock when during page walks for DMA operations. This patch
locks and unlocks the mmap lock for page walking.

The replacement of <asm/pgrable.h> with <linux/pgtable.h> made the include

Patch series "mm: consolidate definitions of page table accessors", v2.
The low level page table accessors (pXY_index(), pXY_offset()) are
duplicated across all architectures and sometimes more than once. For
instance, we have 31 definition of pgd_offset() for 25 supported
architectures.
Most of these definitions are actually identical and typically it boils
down to, e.g.

These definitions can be shared among 90% of the arches provided
XYZ_SHIFT, PTRS_PER_XYZ and xyz_page_vaddr() are defined.
For architectures that really need a custom version there is always
possibility to override the generic version with the usual ifdefs magic.
These patches introduce include/linux/pgtable.h that replaces
include/asm-generic/pgtable.h and add the definitions of the page table
accessors to the new header.
This patch (of 12):
The linux/mm.h header includes <asm/pgtable.h> to allow inlining of the
functions involving page table manipulations, e.g. pte_alloc() and
pmd_alloc(). So, there is no point to explicitly include <asm/pgtable.h>
in the files that include <linux/mm.h>.
The include statements in such cases are remove with a simple loop:

Currently, the log-level of show_stack() depends on a platform
realization. It creates situations where the headers are printed with
lower log level or higher than the stacktrace (depending on a platform or
user).
Furthermore, it forces the logic decision from user to an architecture
side. In result, some users as sysrq/kdb/etc are doing tricks with
temporary rising console_loglevel while printing their messages. And in
result it not only may print unwanted messages from other CPUs, but also
omit printing at all in the unlucky case where the printk() was deferred.
Introducing log-level parameter and KERN_UNSUPPRESSED [1] seems an easier
approach than introducing more printk buffers. Also, it will consolidate
printings with headers.
Introduce show_stack_loglvl(), that eventually will substitute
show_stack().

Working on the OpenRISC glibc port I found that sometimes clone was
working strange. That the tls data argument sent in r7 was always
wrong. Further investigation revealed that the arguments were getting
clobbered in the entry code. This patch removes the code that writes to
the argument registers. This was likely due to some old code hanging
around.
This patch fixes this up for clone and fork. This fork clobber is
harmless but also useless so remove.

The function show_trace_task() was removed during linux 2.5 development
and replaced with show_stack(). This was never impemented for openrisc
but must have got in via copying from another architecture. Just remove
it.

Previously copy_thread_tls was copy_thread and before that something
else. Remove the documentation about the regs parameter that didn't
exist in either version.
Next, fix comment wrapping and details about how TLS pointer gets to the
copy_thread_tls function.

The mm_walk structure currently mixed data and code. Split out the
operations vectors into a new mm_walk_ops structure, and while we are
changing the API also declare the mm_walk structure inside the
walk_page_range and walk_page_vma functions.

Pull force_sig() argument change from Eric Biederman:
"A source of error over the years has been that force_sig has taken a
task parameter when it is only safe to use force_sig with the current
task.
The force_sig function is built for delivering synchronous signals
such as SIGSEGV where the userspace application caused a synchronous
fault (such as a page fault) and the kernel responded with a signal.
Because the name force_sig does not make this clear, and because the
force_sig takes a task parameter the function force_sig has been
abused for sending other kinds of signals over the years. Slowly those
have been fixed when the oopses have been tracked down.
This set of changes fixes the remaining abusers of force_sig and
carefully rips out the task parameter from force_sig and friends
making this kind of error almost impossible in the future"

The openrisc DMA code supports DMA_ATTR_NON_CONSISTENT allocations, but
does not provide a cache_sync operation. This means any user of it
will never be able to actually transfer cache ownership and thus cause
coherency bugs.

As synchronous exceptions really only make sense against the current
task (otherwise how are you synchronous) remove the task parameter
from from force_sig_fault to make it explicit that is what is going
on.
The two known exceptions that deliver a synchronous exception to a
stopped ptraced task have already been changed to
force_sig_fault_to_task.
The callers have been changed with the following emacs regular expression
(with obvious variations on the architectures that take more arguments)
to avoid typos:

Nobody has actually used the type (VERIFY_READ vs VERIFY_WRITE) argument
of the user address range verification function since we got rid of the
old racy i386-only code to walk page tables by hand.
It existed because the original 80386 would not honor the write protect
bit when in kernel mode, so you had to do COW by hand before doing any
user access. But we haven't supported that in a long time, and these
days the 'type' argument is a purely historical artifact.
A discussion about extending 'user_access_begin()' to do the range
checking resulted this patch, because there is no way we're going to
move the old VERIFY_xyz interface to that model. And it's best done at
the end of the merge window when I've done most of my merges, so let's
just get this done once and for all.
This patch was mostly done with a sed-script, with manual fix-ups for
the cases that weren't of the trivial 'access_ok(VERIFY_xyz' form.
There were a couple of notable cases:
- csky still had the old "verify_area()" name as an alias.
- the iter_iov code had magical hardcoded knowledge of the actual
values of VERIFY_{READ,WRITE} (not that they mattered, since nothing
really used it)
- microblaze used the type argument for a debug printout
but other than those oddities this should be a total no-op patch.
I tried to fix up all architectures, did fairly extensive grepping for
access_ok() uses, and the changes are trivial, but I may have missed
something. Any missed conversion should be trivially fixable, though.

If we want to map memory from the DMA allocator to userspace it must be
zeroed at allocation time to prevent stale data leaks. We already do
this on most common architectures, but some architectures don't do this
yet, fix them up, either by passing GFP_ZERO when we use the normal page
allocator or doing a manual memset otherwise.

Use the for_each_of_cpu_node iterator to iterate over cpu nodes. This
has the side effect of defaulting to iterating using "cpu" node names in
preference to the deprecated (for FDT) device_type == "cpu".
This also fixes a leaked reference for cpus node.

The cache maintaince in the sync_single_for_device operation should be
equivalent to the map_page operation to facilitate reusing buffers. Fix the
openrisc implementation by moving the cache maintaince performed in map_page
into the sync_single method, and calling that from map_page.

openrisc does all the required cache maintainance at dma map time, and none
at unmap time. It thus has to implement sync_single_for_device to match
the map cace for buffer reuse, but there is no point in doing another
invalidation in the sync_single_cpu_case, which in terms of cache
maintainance is equivalent to the unmap case.

Originally in patch e6d20c55a4 ("openrisc: entry: Fix delay slot
detection") I fixed delay slot detection, but only for QEMU. We missed
that hardware delay slot detection using delay slot exception flag (DSX)
was still broken. This was because QEMU set the DSX flag in both
pre-exception supervision register (ESR) and supervision register (SR)
register, but on real hardware the DSX flag is only set on the SR
register during exceptions.
Fix this by carrying the DSX flag into the SR register during exception.
We also update the DSX flag read locations to read the value from the SR
register not the pt_regs SR register which represents ESR. The ESR
should never have the DSX flag set.
In the process I updated/removed a few comments to match the current
state. Including removing a comment saying that the DSX detection logic
was inefficient and needed to be rewritten.
I have tested this on QEMU with a patch ensuring it matches the hardware
specification.

handling code and thus careful code review.
Also not included is the work to shrink the in kernel version of

Most mainstream architectures are using 65536 entries, so lets stick to
that. If someone is really desperate to override it that can still be
done through <asm/dma-mapping.h>, but I'd rather see a really good
rationale for that.
dma_debug_init is now called as a core_initcall, which for many
architectures means much earlier, and provides dma-debug functionality
earlier in the boot process. This should be safe as it only relies
on the memory allocator already being available.

Filling in struct siginfo before calling force_sig_info a tedious and
error prone process, where once in a great while the wrong fields
are filled out, and siginfo has been inconsistently cleared.
Simplify this process by using the helper force_sig_fault. Which
takes as a parameters all of the information it needs, ensures
all of the fiddly bits of filling in struct siginfo are done properly
and then calls force_sig_info.
In short about a 5 line reduction in code for every time force_sig_info
is called, which makes the calling function clearer.

Call clear_siginfo to ensure every stack allocated siginfo is properly
initialized before being passed to the signal sending functions.
Note: It is not safe to depend on C initializers to initialize struct
siginfo on the stack because C is allowed to skip holes when
initializing a structure.
The initialization of struct siginfo in tracehook_report_syscall_exit
was moved from the helper user_single_step_siginfo into
tracehook_report_syscall_exit itself, to make it clear that the local
variable siginfo gets fully initialized.
In a few cases the scope of struct siginfo has been reduced to make it
clear that siginfo siginfo is not used on other paths in the function
in which it is declared.
Instances of using memset to initialize siginfo have been replaced
with calls clear_siginfo for clarity.
